Patients who order a Home Sleep Test (HST) using insurance are required to complete the test within 45 days of receiving the device.
By choosing to order the HST through insurance, you agree that:
- The HST is provided with the understanding that it will be completed and billed to your insurance upon test completion.
- You received the HST following an initial clinician evaluation. During this evaluation, the clinician provided guidance that the test must be completed within 45 days of receipt.
- The test must be taken and submitted within the 45-day completion window to remain eligible for insurance billing.
If the HST is not completed within 45 days of receipt:
- The order will no longer qualify for insurance billing.
- You will be billed the full cash-pay price of $189.00.
- GEM reserves the right to convert the order to cash pay in accordance with GEM policy and standard HST laboratory order timeframes.
Failure to complete the test within the required timeframe constitutes acceptance of the cash-pay charge and authorizes GEM to bill the patient (you) directly.
